Overview:


The Nightshift Care Coordinator is responsible for the management, development, performance, scheduling and staffing of Care Advocates personnel. These responsibilities include policy oversight, organizational planning, and the direction for the delivery of care, as well as the development and monitoring of standards of care practices.


Physical Requirements:


This position demands good physical and mental health. The Nightshift Care Coordinator will be required to lift, catty, walk, sit, push, pull and work a flexible schedule, must be able to move continuously during work hours and able to lift and/or carry 50 pounds.


Duties:


Manages, develops, and reviews the overall quality of care services provided by The Carlyle House. Conducts daily rounds with all nurse aids and initiates correction action, as necessary.


Directs the care needs of residents according to their care plans and communicate to the Nursing Department any observation of a resident s medical change of condition.


Conducts monthly (and as needed) staff meetings with all shifts for purposes of planning, coordinating, and implementing direct care service policies and procedures.


Communicates with nurse aids providing clear, concise information.


Work closely with Health Care Director and Assistant Director of Nursing to review all reports of accidents/incidents involving nurse aids and resident for completeness, accuracy of content and appropriate signatures and interventions.


Assist with any investigations involving the allegation(s) of abuse, neglect, misappropriation of resident property and take appropriate disciplinary action as necessary.


Maintains up-to-date position control and master schedules.


Assists with completing performance appraisals prior to due date. Responsible to deliver appraisal feedback to the Health Care Director and Assistant Director of Nursing by due date.


Train and develop all new Care Advocate and continue training on current staff.


Supervise Care Advocate staff during normal shifts and meetings. Conduct daily rounds to ensure ADL s of residents are met.


Communicate any change of condition or care needs of a resident.


Conduct monthly Care Advocate meeting to discuss updates, ADL(s) and scheduled training.


Ensure staff is aware of all state and federal regulations, including OHSA.


Responsible for Corrective Action and appropriate follow-up.


Develop Care Advocate schedules and ensure all shifts are covered to properly care for the residents.


Responsible to work the floor if the shift cannot be covered.


Update and review all nurse aids forms to stay in compliance with rules and regulations. Conduct weekly review of care notes.


Provide feedback to ensure care plans are up to date with correct information and followed accordingly with Health Care Director and Assistant Director of Nursing.


Train and develop Care Advocate on the usage of Point of Care in Eldermark.

Estimated Salary: $20 to $28 per hour based on qualifications.
